1. Please send 4 hard copies (signed by the PI and all co-PIs) of the protocol

2. Please send 1 CD containing the protocol document

3. The IRB fee is 2% of the total budget of the project (not exceeding Tk 50000) and student protocol is Tk 15000

4. Please keep an extra copy/ scanned copy of all the documents submitted

5. Please note that your protocol will be reviewed by two reviewers
